EAST PEORIA, Ill. — A state health department report finds a local rehab center erred when a patient there fell, and later died last spring.
In May of 2022, a patient with a long list of pre-existing conditions fell at Fondulac Rehabilitation and Health Care Center in East Peoria.
25 News reports 67-year-old Richard Munson, was taken to the hospital and diagnosed with a broken left femur on May 24, and died May 26, in part as a result of the leg injury, as well as chronic, hypoxic respiratory failure.
The report concludes that staff should have re-assessed his mental cognition and daily assistance needs when he came back from the hospital, rather than let him try to get around on his own.
